CBCM Case Manager (Full Time)

Job Type: Full-Time

Responsibilities

Provides quality case management to include supportive counseling, service coordination/advocacy, and crisis support.  Develops and ensures that each client has a current individual service plan. Ensures that the client has access to transportation services as needed which may involve escorting/accompanying clients in the community. Develops a transition/discharge plan for each client no longer meeting the criteria for the level of care. Ensures that client input is included in every step of the entire treatment and recovery process with a commitment to work with clients/ target population in community based settings.

Requirements

Minimum Bachelor’s degree from an accredited university or college, preferably in social work, psychology, human services or related field with minimum 12 semester credit hours in courses such as psychology, counseling, criminal justice, human services, social work, social welfare, sociology or other behavioral sciences. Must have 1 ½ years of specialized experience in human services with an emphasis on working within public safety system, ex-offenders, parolees, and dual diagnosis.  Access to vehicle and appropriate no-fault insurance with valid driver’s license and clean abstract.  CPR and First Aid Certification (or able to complete at IHS).  Must pass CPI (Crisis Prevention Intervention) training.
