The Case Manager assists families to ensure a successful and healthy living environment. Works collaboratively with individuals in each family to understand in advance their strengths and needs as a family unit. Assists with resolving tenancy issues and provide individualized counseling, provide or work with outside agencies to skill build in the areas of parenting, stress management, and problem solving to further strengthen family dynamics. Ensures case notes are completed in a timely manner according to IHS documentation standards and performs timely follow-up with outside case managers and agencies involved with the support services to families onsite.
Bachelor’s degree in Human Services or related field from accredited university with 12-15 semester hours in social sciences. At least one and a half (1 ½) years case management experience and/or other support services for families with children in homeless services or community support setting. Working knowledge of community resources, public health benefits, best practice solutions for families who are at risk with homelessness. Able to work under pressure and demonstrate commitment to delivering culturally responsive services. Experience with crisis intervention and de-escalation. Computer proficient in Microsoft Office and database applications.
