Intake interview leading to comprehensive assessment and development of individualized service plans for medically fragile homeless individuals. Provide basic medical case management, supportive counseling and referrals of guests to community resources to help develop skills and end homelessness. Emphasis will be placed on securing stable housing placements for guests and ensuring continuity of care. This position requires attention to detail, basic understanding of healthcare processes and management of chronic health conditions. This rewarding position also requires extensive communication with collateral providers including home health agencies, referring hospitals and insurance providers. Ensure documentation of services through progress notes that meet agency standards. Maintenance of clinical records as required by IHS standards a must. Knowledge of behavior management strategies, motivational strategies and compassion for persons served essential.
Prefer at least one year of experience in the delivery of medical case management services with a Bachelor’s Degree preferred with a minimum of 12 semester credit hours in Health or Behavioral Science Coursework (i.e. psychology, social work, criminal justice etc). Favorable experience also includes 2-3 years working in medical or home health settings. Knowledge about healthcare and substance abuse service systems is preferred. Working knowledge of community resources, mental health issues, substance abuse service systems, public health benefits and criminal justice systems to assist clients. Able to work under pressure and handle a large volume of cases. May need to work occasional weekends and evening hours.
